Why can't I see any caller id screen displayed on incoming call (only ringtone) ?

I have Moto G (2nd Gen). Recently whenever someone calls me, I can hear only ringtone but no caller id information is displayed. I can't receive or hung up the call. I need to press the unlock button of the phone to control call. Please suggest me what to do.
