Why is my SD card not being recognized by my phone and my computer?

I've tried almost everything but just to tell you my phone in turbo x and it wont ''see'' the micro sd card. so... just to make sure that the sd card didnt have a problem i put it in an adapter and tried it in the computer. It didnt see it but i researched a little and saw that my version of computer would recognize an sd card like the one i have because its newer than mu computer( my computer is kinda old). Please....im ready to give up but can anyone please help????
