Why is my phone not charging even though the screen says it is?

Whenever I plug in my Samsung Galaxy S4 into the USB to get charged, the screen says it's charging, but the percentage still drains. I figured if I cleared any processes it would help, but it didn't. I ended up turning off my phone to let it charge. It's been an hour and my percentage has only gone up by 5. Is there something wrong with my charger or my battery?

Welcome to Android Central! If you're charging from a computer USB port, charging will always be pretty slow, because the maximum current provided through a USB port is pretty low. If you're actively using the phone, the battery may actually continue to drain, despite being plugged in. It'll just drain a little more slowly.
