How Do I Unfreeze my SD card menu?

[AC Question]

My SD Card menu under Settings > Storage has frozen and will not me select any of the items under it (move media, mount SD card, erase SD card)

About a week ago my SD card was 'unexpectedly removed' and despite my best efforts cannot be read or recognized on my phone (but does work on my computer)

I have reformatted it, reset my phone to factory settings and tried cleaning the SD card. Any suggestions?

Welcome to Android Central! Which phone? It may be that the card is defective or failing. Do other cards work normally in the phone?
