    Do you see the switch or toggle turn back on by itself? Or are you noticing wi-fi still on in the Battery graph, even though you turned it off?

    I go into Wi-Fi settings turn it off with the on/off slider and after 5 minutes or so it comes back on and tries to connect

    See if that happens in Safe Mode, which disables all 3rd party apps: http://www.androidcentral.com/galaxy-note-4-safe-mode
