Do I need a phone to just check my heart rate? I need to keep track of heart rate as I have a heart problem but don't really need any of the other programs. I have a apple 5s phone not a Samsung one. If I have to download the manager I have. a kindle HD and a HP computer.

No, you can buy a heart rate monitor on its own, plus most phones don't come with heart rate monitors and the ones that do I wouldn't trust their readings. Speak to your doctor, they will be able to assist you with choosing a decent heart rate monitor.
