Why does my phone die/shut off even when it has 98% charge.

I can disconnect the charger at 100%, make a 1 minute phone call, send one text, take a picture or log into social media and my phone will just shut off. Then it takes several attempts to get it back on to have it do the same thing even if it says 90%.
