Why is my phone shutting down at 60% charge?

After my samsung galaxy note 4 has been fully charged, when the battery reaches 60-68% it turns off! when i plug in the charger it shows 0% charge then starts charging. Sometimes it also shows 50% charging while plugged in. do i need a new battery as i have factory reset 3 times.
