Nexus 5: Proximity Sensor Not Working After Root

Hi. I am using Nexus5 for more than a year and after expiration of its one year warranty, I rooted it with Cyanogen13 Nightly mod. After a month of usage, I re-installed stock rom to my device and it worked fine. Suddenly few weeks after, its battery life dropped drastically and after two weeks after that, the proximity light sensor stopped working. I've done factory reset after that but nothing seems to be working. Any help please?
