Why is the volume playback on my Samsung Note 4 turning itself down automatically?

It has happened in the past infrequently, but now it is happening every time I go into the gym and start listening to a podcast or something else I am playing. I have to keep turning it back up, and then it turns the volume to off. I am not hitting the volume key except to try and keep the volume from going off. I go into the phone and adjust the settings, but it is still turning itself down automatically. I see several posts on what to do, but I need a specific "go here on your phone settings. Uncheck this box. Check this box." etc., I see so many are having this save issue. Save America and keep the volume up!

If you're using earphones, that's automatic (to keep you from losing hearing) and there's not much you can do about it. (If you keep the volume all the way up when you're using earphones, you will lose hearing, whether you realize it's happening or not.)
