How can completed travel expense forms suddenly become blank

I completed some travel claim forms in September 2015 on my samsung tablet, printed them off and signed them and sent them off recorded deliveryto my then employer, who acknowledged receipt but still hasn't paid me.

Whilst completing a legal form to reclaim the costs, I looked at my saved files which were completed and then uploaded the files to the legal claim form but noticed they were all blank. When I went back to my saved forms all of them were also blank...ie none of the mileage claim information were on the forms (about 9 forms) and I note that prior to me attaching the date of last being amended was2/09/2016 now most say 15/08/2016 and a couple say 16/08/2016. How can this happen
