Unwanted screen appears after making/receiving phone call

[AC Question]

After I receive or make a call, a strange screen appears. It partially covers whichever screen was previously showing (home screen, keypad screen, etc.). It looks like an interactive screen with 7 icons to tap (Harassment, Sell, Bilk, Express Delivery, Intermediary, Takeout, Decoration). However, nothing happens when an icon is tapped. Also, when receiving a call, the top 1/3 of the normal contact screen is overlaid with a green background, blocking the caller info and showing the incoming telephone number. The only way to get rid of the strange screen is to hit the HOME button.

Has anyone else had this problem? I can upload a screen grab if possible. (This is my first Forum)
