why isn't my sd card detecting in my andoid phone?

I have a sandisk 2gb sd card. It was inserted in nokia c2-01 handset when I set password in it. Then i forgot it. After doing a factory reset of phone settings it asks the password. Then i inserted it to a pc but it is not detecting the card. I also tried it on android phone but its not working. It is only detected by the previously mentioned handset and it is showing locked and ask password. Even it can't be formatted without password. Pls reply.....
