I'm using Lenovo Phab Plus Android Phablet. For the last couple of days, whenever I'm taking a call, the screen light turns off as soon as I take my phone near to my ear. Then it becomes difficult to disconnect the call. Every time, I've to ask the other party to disconnect the call. The phone works perfectly fine when the call is answered through a headset or through a speakerphone.

Welcome to Android Central! It's normal for the screen to turn off on a call when you put it up to your year -- that's the proximity sensor working properly. But are you saying that the screen doesn't come back on when you take the phone away from your ear? Does it have a screen protector or a case on? If so, make sure they're not blocking the proximity sensor, which is right near the front-facing camera.
