Moto M microphone and audio jack issue (software)

[AC Question]

My Moto M 's microphone and audio jack sometime does not work properly. This is not a hardware issue as it works again immediately when I restart the phone...But this problem whenever happens , it happens suddenly and becomes very annoying. Anybody can help me regarding this? There no OTA updates also, so I think I can not update the software to correct these problems

*Does it persist in Safe Mode?
Also, with SD card removed?
