Apps disappearing from home screen on restart - Lenovo P2

[AC Question]

I've tried googling this and the general response was to reformat the SD card, reinstall the apps and to factory reset the phone - I've tried all and the same apps are disappearing, with apps also installed to the memory card staying put.

SD card is formatted to be part of the internal memory.

Facebook
Facebook Messenger
Space Marshals 2
Square Instapic

Apps that stay:
Instagram
Spotify
Whatsapp
Score Hero
Wunderlist
Maps.me
