What causes the following to happen?

So, we've got an lg g6. About to be on the 4th one now. The first two times the phone had to be replaced, it had been plugged into a work truck to charge, the screen went yellow, then shut off and wouldn't turn back on. This time it was the same thing, but it was in our personal truck where we've charged the phone before. Now we're thinking it's the adapter for the trucks that's caused this (the adapter goes back and fourth between the two trucks) so, I'm wondering what can cause an adapter to start doing this? We're scared to even try charging the next one in the truck now even with a new adapter.

Welcome to Android Central! What brand and model is the charger? Unfortunately, there are USB-C chargers that do not follow the proper specs and increase the danger of damage to the phone. Benson Leung, a Google engineer, has made it one of his priorities to educate people on this.
