Galaxy Note 4 loses SD card after a while. When i reboot the phone, the card is there. However, I'll try to take a photo, with camera set to the SD, but then it is often gone and it switches back to internal storage for photos. This is a Note 4 I've had for several days in an insurance replacement from t-mobile. The card is a 64 GB card from PNY.

Welcome to the forum. I've seen the same thing with my new S8+ and a different SD card. Sometimes phones prefer one make over the other, but what I've seen personally and here on the forums, SD use is still iffy. You might try a different card if you have one, otherwise I have no golden bullet for you. Sorry.
