What is causing the "Select line to call from" prompt to always popup when making a call

What is causing the "Select line to call from" prompt to always popup when making a call. I never use to get this so I assume its an application that is interfering with me making calls. The prompt also lists my cell number but no other line is listed which is strange because if only one option can be selected then why even give a choice.

Welcome to Android Central! Which phone? Is it dual SIM? Or do you use Google Voice as well?

Its a LG 5 phone single SIM card, I do use google voice and I use to have T-Mobile digits installed which I have since removed. I think I started get this prompt when I had T-Mobile digits installed

If you open your Google Voice app, is it set to ask you each time which line you want to use whenever you make a call? What happens if you set it to default to your Google Voice number? Perhaps Digits caused that Google Voice setting to reset.
