I'm trying to get all the files off the SD card that I had in my old phone (galaxy s4) to my new phone (galaxy note 4).

Unfortunately, when you move a game to the SD card it doesn't move EVERYTHING. You'll need to reinstall the game on the new device, then move the app data to the new device

You could use Helium to back up the apps and their data (it only backs up data by default - change that to back up the app as well), copy the carbon folder to your PC, plug in the Note 4 and copy it to the Note 4, install Helium on the Note 4, run it and restore the apps.

Or, if you don't need any data moved, you can use App Backup & Restore to back up on the S4, then use your PC to copy the App_Backup_Restore folder to the Note 4. The apps are all saved as .apk files, so tapping any of them will install them on the Note 4.
