Not only do I get an "oops files lost" error message but it shows a girl with her shoulders shrugged. I have very few apps on my phone. Did I get a virus or is my phone hacked? Does anyone else have this happen?

Knowing the make and model of the phone might help us to help you.

Welcome to Android Central! I've never seen nor heard of anything like that, so it's likely from some app. Does it show up as a popup window? If it shows up like it's a separate app, then the next time you see it, tap the Recent Apps button, and what app name is on that minimized window.

Long-press the notification and see what choices you get. If one of them is App Info, tap that and see what app sent the notification.

Try deleting the Flashlight app that you downloaded. I think it was the Galaxy Flashlight by Bejing Fingerfly tech. Worked for me.

Ugh, flashlight apps. 3rd party flashlight apps are more likely to have adware, completely unreasonable permissions (it should only be a Camera permission, since that's what is needed to control the camera LED flash), and frank malware. Every modern Android phone should have a flashlight toggle in the Quick Settings -- use that instead.

Boot into safe mode and confirm the app suspicion.
