I have removed all Notifications except for Gmail and Maps. Still randomly does ring tone and adjusts volume to max even though I set it to min. I see a number of threads where people see the problem, but I don't see any answers. I have not recently added new Apps.

That can be due to a number of problems - ghost touches, a component on the motherboard breaking down, an app going rogue, and probably a few I'm not remembering at the moment. If you don't have a technophile friend who's familiar with Android, a repair shop may be your only out.

(Try setting your default ringtone to something else and see if the random ringtone becomes the new one or stays the old one - that will help in diagnosing it. If it's still the old one, some app is probably causing it. If it becomes the new one, the phone app may be triggering without an incoming phone call.)
