    Welcome to Android Central! Why not just call Optus? Are you trying to carrier-unlock the phone, or did you forget the lockscreen PIN/password that you created?

    The only time you need the PUK is when you tried the unlock code too many times and it was the wrong one. It doesn't unlock the phone.

    As was said, call your carrier for the PUK (or bring the phone in).
