Between calls are you listening to music, watching videos, what are you doing prior to the calls?
Do you increase the volume for other activities between calls?
Do your headphones have volume controls?
I do not believe Samsung has a control to lock the volume. With Bluetooth they have a volume limiting control that must be acknowledged to exceed which I'm not too sure about with wired headphones but if you have already acknowledged and exceeded it to listen to music it won't reset until the phone is restarted.
I've heard talk of an app at the Play Store, Sound Control Pro for one, that gives more control and a widget.
