How are they set?

Is it from the sign up date? Does Fi allow for user choice? Is it driven by the NPA-exchange (area code and first 3 digits ) of the telephone number?

Can they (the dates) be changed?

It's user choice - because it starts when you install the SIM card and activate the service. Kind of doubt it changes after that...unless you suspend and then restart the service.

You receive a bill on the day you activated your service. In my case, I activated on the 3rd, so I get my statement on the third of every month. They automatically charge your payment roughly 10 days later; my last payment was Dec 14.

I'm not sure if they can alter the day they create the statement or your payment date, you'll need to call and ask.
