I've turned off everything I could find re:DND, but it keeps turning itself on. Please see attached. I've also tried the Do not disturb toggle app, but it won't work on this phone.

What andriod version its running, this one of government funded phones ? They usually have all types of stuff on them like ads , apps can't be deleted . In that screen shot it mentions UI controls it somehow

It's still worth testing in Safe Mode, because these Lifeline phones (especially ones from UMX) are notorious for having preinstalled malware that automatically installs other apps without you necessarily knowing about it.

I didn't notice the DND did not turn itself on in safemode. In regular mode, I did notice the DND also gets turned on again when charging.

That suggests some 3rd party app is causing DND to turn on. You might have to uninstall apps one by one until the problem goes away, since it can otherwise be difficult to pinpoint the culprit without having any prior suspicions.

Haven't tried safemode because I recently reset to factory state and have only one downloaded app--Geico.
Even with Geico app as only app downloaded , long press it tap info > go to permissions do you see anything say enabled to modify system?

Glad the problem seems to be solved, but it's odd that Chrome or the Play Store would do this. Let us know if it comes back.

Hello again. Spoke too soon. After disabling those apps and two days of no DND -- it's back. How insidious that a system app, which it does not identify, would cause this nuisance.

UMX is notorious for including relatively shady things in the system, like adware, so it might have to do with that. Not sure what else to suggest at this point, sorry!
