Muted app notifications (Rhapsody & iHeartRadio) from Notification (Status) Bar

Unintentionally muted Rhapsody quick access (play pause next back) from the notification pull-down screen.

I long pressed the widget in the notification screen and was presented with an option to look at app info and to mute. Pressed mute, and the quick access screen disappeared. Can not figure out how to get it back.

Tried Settings | Notification Manager | App Notifications Rhapsody & iHeartRadio
Tried Settings | Apps | Rhapsody | Show Notifications Check Box

No luck...

Android 5.0.2
Cyanogen OS Version 12.0-YNG1TAS2I3

I tried the same thing on my phone and only got "App Info." I'd recommend going to Settings> My Device> Sound> Volume and make sure that your notifications volume is turned up.

And just to clarify, are your notifications muted or are you also unable to hear sound from either of those apps at all (even when music is playing)?
