This as hapened a few times in Jelly Bean and now in KitKat.

I have files in my download folder, files in folders in my download folder. I know they're there I can see them using ES File explorer on the phon, But when the phone is connected to my windows 7 PC as a "media device" and I browse with windows explorer some files and folders are missing - what gives ?

The download folder has a .NoMedia file, tried removing that disconnecting the phone and browsing again - no joy.
Tried renaming the folder \ files on the device - still no joy.

It not just media files that im missing, its .mkv, .avi, .pdf .srt, .txt and some folders

This has happened a few times over the last few months. Is not huge hassle, but if there a reason \ fix Id like to know what it is,
    anyone ?

You need to wait for the files to be indexed. Reboot your phone and wait 3minutes, only then connect your device to the PC to see the new files.
