Icons disappear

Everytime i restart or turn off and on, the icons that I moved to my ad go away. Well the app is still where i put it but the icon is a green android box, they look normal in my drawer.

Is there something to fix this or is this a bug?

Try to switch "Scenes" in the Personalize Menu to a different scene and then back to the one you had and see if that works... if so thats a problem we are all having with apps on sd card and will hopefully be fixed with the next ota update.
