How do I stop all my outgoing calls from defaulting to speaker phone?

Every time I make a phone call, my EVO does it in speaker phone mode, and having to take it off speaker mode isn't the easiest and quickest thing to do. Is there a way to have the phone answer and make calls in handset mode? I've searched around but everyone else who has the same problem has not yet been able to fix this.

Do you have the phone plugged into a charger when that happens? I ask because that is what is supposed to happen when connected to a car mode charging cable. Car mode cables have pins 2&3 shorted together.
