Is there malware on my HTC One M8???

First, just to confirm, I've been hacked before on this device and I've since reset it at least 5 times. I've recently installed the Lookout Mobile Security app and I've gotten a free trial of sorts that has scanned my phone for viruses, saying that there are none, but I still have some concerns. My HTC one M8 has a second phone app installed with a blue phone icon. I looked into the app's details and found that it has access to almost everything on the device, but I cannot deactivate, uninstall, or clear the cache of the app. Is this app supposed to be on here?
