    For about a month now I have been having issues with my LG D415 (L90) occasionally not recognizing my T Mobile SIM card, and being stuck in headphone mode after a normal restart.

    This happened most recently last night (Mon. 8/31) and after 2 factory resets and countless attempts to reseat the SIM card the problem persisted. This morning I tried one more restart before taking my phone to the T Mobile store and the phone now recognizes the card.

    Additionally, my phone displayed a "Firmware Update" screen out of the blue last night. I'm not sure why it chose to do this while I was nowhere near a WiFi signal or my laptop but it did. I'm not sure if that is related to my problem or not. This screen went away after I restarted my phone.

    Do I need to go get a new SIM card either way or is this a problem with my phone itself? Thank you for your time and suggestions!

    Welcome to the forums. I would try a new SIM card. See if that helps.
