Yes to both. My MHL adapter is from Monorpice and the OTG cable is generic I think from Amazon.

None that I can tell. The case fits snug, but not excessively so. If anything, it is just slightly more difficult to put on than it would be with the standard battery. IMO, the case fits just fine.

I have the Otterbox Commuter for the Vzw Gnex. It fits well. I have the extended battery in mine (2100 mAh) and the case still fits.
