I used is successfully : t-mob, not rooted, everything pretty much "out of the box". (For now...). No clue about my Sim, but its over a year old and was just cut.

Not sure which Square Trade policy you were looking at, but I just bought one through amazon, covers phones valued at 300-400, $50 deductible, all hazards (kids not excluded!), $80. Agreed that $125 for a $100 deductible on a $299 device really doesn't make sense...but for $80 and a low...
