I found the issue. On the contact , the option for 'route to voicemail' was enabled.

I am using a Pixel 3XL. Phone just started blocking my wife's calls all of a sudden. I'll either see a random missed call from her # without the contact info showing , or it will just block the call entirely. I am not using any 3rd party apps, the number is not listed in the blocked # section...
