I've had some Moto phones that are picky about the cord you're using. Sometimes using other USB cords can cause the boot-up you described. If this is happening with the supplied charger, consider swapping out.

Not all Verizon plans charge extra for tethering. Any recent plan will have tethering included in your data usage. (It just uses your allotment)

My Droid RAZR MAXX HD has just started this behavior over the last couple weeks, as well. I have not noticed it happening in the car yet, but it mostly happens just talking on the phone. Suddenly switches over to the speaker for no apparent reason.

Can you tell us what "power cycling does nothing" means?

Um.. No. "The rest of us" are not having issues with 4.2.1, either.

I would believe that the books are tied to whichever account originally PAID for them on whichever service, such as PlayBooks, or Amazon Kindle App, so, at least on the Nexus 7 or maybe other JB 4.2 devices, you could put a second account (your daughter's) on your Nexus 7 to get the books on there.
