Large 'X' just below the notification bar
It happens if you long press the app to remove it and you don't actually bring the app up to the remove option. When the X sticks just long press the app again and let go, without removing the app and the X will disapear. This occurs on Nova Launcher for me. -

People not receiving my picture messages?
I've had the same problem with my Moto X. I tried to send a picture that I took with my camera. The recipient texted me back with the above error message, I had to crop the picture and resend and it worked.
