If all else fails - weird fix but worked for me after trying ALL fixes here. I dropped my phone and I think that's where it started. I read about this fix and by golly it worked. I simply re dropped my phone on the hard kitchen floor from about 5 inches - my original drop was about 4ft. Yup...
