Create a new APN similar to what I have, select it and your data will go offline for about 2-3 seconds and come back up with the new APN. No need to reboot and all should be working over mobile network. Cheers

To add with regards to the Wifi issue, Samsung is well aware of it and they have rolled out the fix for Verizion and working on it for ATT, when the carriers actually push the fix well you all know how quickly ATT updates the software OTA. I wouldn't expect this to be fixed until M comes out...

The mobile network issues has nothing to do with your phone, it is a carrier issue. The wifi problem is a software bug that Samsung is aware of and is working to fix and it is not resolved in 5.1.1. No there is no app to fix this, if you want it fixed now you must change your APN.

It has been fixed in Silicon valley (SF/ Cali) a week ago, it is being rolled out across the nation, I myself in So Cali are waiting so you can be sure it will take a while but it is being slowly rolled out.
