I use Evernote. With it, you can write notes, attach several types of media to a note, take a picture/snapshot, record audio/dictate. There's also a handy sync feature.

The warning has something to do with the front face camera but I didn't put any thought into it, mainly because I don't take pictures with the front face camera :)

Try Settings > Language and Input, scroll down to the Speech section, tap "Text-to-speech output" and then enable "Driving mode" :) Edit: Oh, and you can select what kind of stuff you want Android to read aloud if you tap "Driving mode". Forgot it because I actually don't own a car!

Currently, same as OP. People just stare when the phone rings :p I had Skrillex - First of the Year for a while, the part when the little girl screams "CALL 911 NAO!" and then goes wubwubwub. People were staring even more :D
