Do you happen to have Gasbuddy on your phone? It wants access to bluetooth as a permission and I found that it became my #1 battery consumer. I was also having problems with apps losing permissions like you. Since uninstall about a month ago I have had no problems with either.

This may apply or may not apply. The Verizon Upgrade Assistant will not work if the phone is plugged unto a USB 3 port. Are you plugging into a 2 or 3 port?

You could install the Google Now launcher. I think that has the vertical app drawer you are looking for and have heard that is the one that comes with 100% vanilla MM.
