I had the same problem with a Verizon G4. What finally ended up working was enabling usb debugging (similar to OP's first screen capture), then plugging the phone into the computer. It briefly shows up as a CD drive. Open it and then open the Tools folder. There should be an *.exe file there...

Don't think there's any LTE switched on in LA yet. Check s4gru.com for the latest. Last I checked, there were two 4G LTE towers that had been completed (as of 8/12 or so). Looks like October rollout, maybe late September at the earliest.
