I had the same issue as well. I sent an email to Prizelogic and they told me to mail in the forms, instead of trying to upload them on the website since it wasn't recognizing my IMEI and Wifi Mac address numbers :-( Now that the Galaxy Note 4s have officially been released, I'm going to try it...

Tomorrow will be an interesting day for sure, with so many of us still not 100% sure that our Note 4s will come. I think I'm going to pretend to do some yard work in front of my house, all the while stalking the UPS driver lol! He always comes late! UGH! I can't wait to see all your Note 4 pics...

Signed up for shipping updates via UPS today, and just received a text with tracking number for my Note 4. Still nothing from T-Mobile though. I created a UPS account so that I can be notified whenever an item is shipped to my home address ☺ Posted via the Android Central App
