hmm..exact same crap happening to me had the 32gb ultra sandisk for 2 months, no issues, jellybean rooted. Change to v36 omega rom, then stared getting blank sd card message. Coincidence maybe, haven't tried changing back to stock jb to see if it makes a difference but i think it wont after...
