My 16gb Lexar stopped being recognized in my Galaxy S4. It seemed to be unmounting and remounting itself randomly. Files will appear and then disappear again. I am not sure if this was caused by me removing the card without unmounting it the first time, maybe that screwed it up. Now it can't...
