    if u have a Samsung phone, using dialer type *#0808#, & select mtp + adb for usb connection.. resolved my issue in no time, was suffering for almost a month with no connection issue..

    Solved.. Same issue on Note 5.. *#0808# change usb cable.mode mtp +adb and should work..
