Yeaah...I just figured it out. Well...not a fix but figured out what it is. I flashed my phone to the stock rom when it was still Android 4.04 and the camera was fine! As soon as I updated it, bam! It started happening again. So it's all software related. Grrrr! Sucks when updates mess things...
