I had the same issue as well but rooted my phone and i have had no issues since

signal strength sucks for this phone the phone i love but it has alot of errors even after the ota i ended up rooting my phone and then it was bug free and had no issues except for the bad 3g service and i had the evo 3d before then and service was still bad but no where as bad as this and on...

its been happening to my phone ever since the new ota sometimes not even when charging, but basically its like it attempts 3 or 4 times then finally reboots dont think anything can be done about it on your end , im about to root my phone hoping that none of these issues occur
