Didn't work on my Note 4 w 5.0.1 but I got it after some doing...hope this helps someone searching and finding this thread like I was. Unpair then go into Settings>Applications >All>Bluetooth and clear Data & Cache. Only the unpaired devices will be gone when you go back into Bluetooth in Settings
