Looks like Beautiful widgets (at least the weather icon in the clock looks like it). Here's what mine looks like currently using Beautiful Widgets (sorry for the pic quality, haven't rooted so can't take screenshots):

The stock clock w/weather is an HTC exclusive widget. The original Fancy Widget duplicated it, HTC complained and they changed it. You can pretty much replicate it with one of the many Beautiful Widgets skins, though.

You can also download Fancy Widget from the market (it's free) and use that (digital clock with weather).
